wrong medical record and missed diagnosis
Last October I was admitted with an urine infection to Urology at Blackburn. After 3 days I was discharged with antibiotics. A week later I went to A & E as my pain had retuned. This time they X rayed me and found a kidney stone. Subsequently I had a scan at Burnley and they confirmed that the stone was passed and I was discharged. I recently noticed that a new medicine was appearing on my repeat prescription form. When I was at my GP I asked about this. He informed me that the letter from the hospital on my records said that I had been prescribed this drug for a prostrate condition. This is incorrect. I have never been told I have such an issue and I have never been given this additional drug. The GP asked questions as a check and then noted my records that this was incorrect. I am concerned that a false history has been placed on my records which could have lead to me taking the wrong medication. Also Blackburn did not identify that I had a kidney stone until I was readmitted to A and E. They should have x rayed on the first admission. they should have communicated better with me as the patient. They seem to have put erroneous information on my medical record.
